ConsultBridge Terms

These ConsultBridge Terms apply to ConsultBridge by FabrixMed Systems LLC ("FabrixMed", "we", "us", or "our"). They supplement the FabrixMed Terms of Service, Privacy Policy, HIPAA Notice, Acceptable Use Policy, and any signed agreement or BAA.

1. What ConsultBridge Is

ConsultBridge is a provider-to-specialist consultation and care coordination platform. It helps authorized treating providers and healthcare organizations route structured clinical questions, diagnostic data, images, labs, care context, video sessions, and related information to specialists or care teams.

ConsultBridge supports workflows such as urgent triage, specialist consults, asynchronous interpretation, diagnostic review, and care coordination. It is not a direct-to-consumer telehealth marketplace, emergency medical system, EHR replacement, insurer, pharmacy, or staffing agency.

2. Eligible Users

ConsultBridge may be used by licensed healthcare providers, authorized clinical staff, specialists, facilities, health systems, provider networks, payers, and other organizations approved by FabrixMed. Patients may interact with ConsultBridge only when invited or directed by an authorized care team or organization.

3. Clinical Responsibility

FabrixMed provides workflow technology. Treating providers and their organizations remain responsible for patient evaluation, diagnosis, treatment, referrals, follow-up, documentation, escalation, and emergency decisions. Specialist responses are intended to support clinical decision-making and must be reviewed by the responsible treating provider.

Urgent triage features are workflow tools and do not replace emergency protocols, on-call coverage, 911, local emergency departments, or direct clinician-to-clinician escalation when clinically required.

4. PHI and Records

ConsultBridge may process PHI. A BAA must be in place before production PHI is submitted. Users must enter accurate, relevant, and minimum necessary information and must not upload data they are not authorized to share.

Healthcare organizations remain responsible for their official medical records, patient notices and consents, retention obligations, and EHR documentation. ConsultBridge records, messages, images, labs, video metadata, and consult notes may need to be copied or reconciled into the customer's designated medical record system.

5. Provider Credentials and Availability

Providers and specialists must maintain active licenses, credentials, malpractice coverage, and permissions required for the services they provide. FabrixMed may verify or request credentialing information, but each provider and organization remains responsible for ongoing compliance.

FabrixMed does not guarantee specialist availability, response time, clinical outcome, referral acceptance, authorization approval, reimbursement, or payer coverage unless a signed agreement expressly states otherwise.

6. Payments

Fees, consult rates, remittance, platform fees, and payment timing are governed by the applicable order form, provider agreement, customer agreement, or checkout flow. Payment workflows may rely on third-party payment processors.

7. Prohibited Use

You may not use ConsultBridge to provide care without required licensure, submit false clinical or credentialing information, delay emergency care, route PHI through unauthorized channels, misuse images or diagnostic data, scrape the platform, or interfere with system security.

8. Suspension

FabrixMed may suspend or restrict access if use creates a patient safety, security, privacy, credentialing, legal, or payment risk.
